How to Send Us a Referral?

Referrers can send us referrals for their patients requiring a neurological assessment, electrodiagnostic testing or both by accessing the Specialist Referrals weblink below:

Information on Referrals – Please Include:

  • Details of the patient’s symptoms and clinical examination along with your provisional diagnosis
  • Pertinent question/s or issue that the neurological assessment should address and answer
  • Copies of the relevant investigations and reports
  • Relevant correspondences from other specialists
  • Copies of patient’s neuroimaging studies along with reports
